📝 Student Registration Form

(For Playgroup to Matric Classes)

Welcome to Superior Mind School and Academy! We are now accepting registrations for new admissions from Playgroup to Matric.

🎯 Who Can Apply?

We welcome students in the following classes:

  • Playgroup / Nursery / Prep
  • Grade 1 to Grade 8
  • Grade 9 & 10 (Matric – Science & Arts)

📋 Registration Process:

  1. Visit the School Office
    📍 Police Choki Wali Gali, Thokar Niaz Baig, Lahore
    🕘 Timings: 8:00 AM to 1:00 PM (Monday to Saturday)
  2. Fill Out the Registration Form
    • Complete student & parent information
    • Class applied for
    • Academic history (if any)
  3. Submit Required Documents:
    • 📸 1 Passport Size Photograph of Student
    • 🪪 Photocopy of B-Form (Student)
    • 🧾 Photocopy of Father’s CNIC
    • 📞 Valid Contact Number of Parent/Guardian
  4. Attend an Interview or Entry Test 
    • Conducted for Grade 1 and above
  5. Confirmation of Admission
    • Final decision made by school administration
    • Once approved, pay the admission & monthly fee

💼 Important Notes:

  • Incomplete forms will not be accepted
  • Admissions are granted based on seat availability
  • Submission of form does not guarantee admission
